MOCSA StoryShare – INDIVIDUAL RELEASE 

I, the undersigned person, attest that I am 18 years of age or older and have granted permission to the Metropolitan Organization to Counter Sexual Assault (MOCSA) and its contractors and employees to use my audio or video documentary, writing, visual artwork, graphics, or photography, credited to me and contributed to MOCSA, in connection with this storytelling/civic engagement project and in connection with advertising, publicizing, exhibiting and exploiting the project, in whole or in part, by any and all means, media, devices, processes and technology now or hereafter known or devised in perpetuity throughout the universe. 

I understand that you are not obligated to use the work and that if you do elect to use the same, you may change, edit, add to or subtract from the work as you see fit. I understand that while I also retain copyright to my own documentary, I hereby authorize MOCSA to utilize my work in any way you so choose. This release is given without the promise of compensation. I understand that the primary purpose of the StoryShare project is a sexual assault awareness campaign.

Your exercise of such rights shall not violate or infringe any rights of any third party.

I hereby release MOCSA and its contractors and employees from any and all claims and demands arising out of or in connection with such use, including, without limitation, any and all claims for invasion of privacy, infringement of my right of publicity, defamation (including libel and slander), false light and any other personal and/or property rights.
